Picture ID of the person applying for financial assistance.
Social Security documentation for all household members:
- Social security card, original, picture or copy of the card or printout from SSA with name and number.
- or W2 form from employer that includes the entire social security number, any year.
- Non-Citizens will need to provide proof of identity documentation. This may include W7, ID or passports from the country of origin or birth certificates.
